Submitting Documentation and Key Dates

All immunization records and associated documentation must be submitted online through University Health Care's confidential Patient Portal. Please note: You must still submit your vaccination records through the Patient Portal, even if you have already provided proof separately for clinical placement purposes.

    • Fall 2025 Semester Deadline: August 1, 2025
    • Spring 2026 Semester Deadlines:
      • Law School: December 1, 2025
      • Undergraduate, Graduate, and Doctoral students: January 9, 2026
    • Summer 1 2026 Session Deadline: May 1, 2026
    • Summer 2 2026 Session Deadline: June 1, 2026
    • Log on to the Patient Portal.
    • Select the "Forms & Immunization Requirements" tab from the left navigation menu.
    • Any missing immunizations will be listed on this page.
    • A status that reflects as "Compliant" in each section (Measles, Mumps, Rubella, Meningitis) means you are in compliance with your immunization requirements.
    • Log on to the Patient Portal.
    • Select the 鈥淔orms & Immunization Requirements鈥 tab from the top navigation menu.
    • Then select the 鈥淯pdate option for each requirement listed.
    • Select the 鈥淯pload鈥 tab from the top navigation menu, then upload your supporting documents (i.e., immunization records, blood titer results, etc.).

    Please allow up to seven business days for immunization document submissions to be reviewed by Point and Click. Students will be notified by email when documents have been reviewed.

    • Immunization records from your health care provider
    • High school immunization records with proof of vaccination
    • Immunization records from any previously attended institutions with proof of vaccination
    • A Student Immunization Form signed and stamped by your doctor. You may find this form in the forms section of the Patient Portal.
    • Physician documented proof of disease (proof of disease is not acceptable for Rubella) A dated lab report reflecting blood tests (a Blood Antibody Titer) proving immunity to measles, mumps, and rubella.

    To avoid processing delays, be sure your record has your Name, Date of Birth, and that it is signed/stamped by your treating healthcare provider. Once you have submitted your documentation, processing may take up to seven days.
